St. John prekindergarten applications ready

RESERVE — The St. John the Baptist Parish Public School System is accepting applications for its prekindergarten programs for 3- and 4-year-olds for the 2015-16 school year.
A child must be 3 by Sept. 30 to be eligible for the 3-year-old program or 4 by Sept. 30 to be eligible for the 4-year-old program.
Applying early is recommended.
Applications are available at parish elementary schools, the St. John the Baptist Parish School Board Office, the St. John Child Development Center and at www.stjohn.k12.la.us under Links.
Applications should be submitted in person at the St. John Child Development Center, 117 Stebbins St., Garyville, or West St. John Elementary School, 2555 Highway 18, Edgard.
When applying, the following are needed:
• Income verification: Family income must be verified before a child’s eligibility for Head Start can be determined.
Documents that can be used to determine income are income tax form 1040 for the previous calendar year; W-2 statements for all working family members for the previous calendar year; recent pay stubs for all working family members; documentation showing current status/amount received from SSl/Social Security; foster care documentation, such as a letter for foster child; documentation of child support/alimony; documentation of unemployment compensation; written statement from current employer(s); written statement of family support from person supporting the child; or income declaration form.
• Child’s birth certificate.
• Child’s Social Security card.
• Child’s medical insurance or Medicaid card.
• Documentation of child’s disability (IEP or IFSP), if applicable.
• Child’s immunization record to show the child is up to date with all shots.
